Psychiatric Disability Evaluations
Completing medical disability paperwork can be a complex and overwhelming process. Errors or inaccuracies may result in delays or even denials of the financial assistance you need. Psychiatric Disability Evaluations simplify this process by ensuring all information is accurate and compliant, giving you confidence and peace of mind throughout your application.
Gotham Psychotherapy supports patients in completing disability-related documentation. Individuals with psychiatric disabilities requiring verification may schedule an appointment with our mental health disability specialist, Elaina Kokinakos, MHC-LP. As required by law, documentation from a qualified mental health professional serves as valid proof of impairment.
We provide official statements and letters to verify conditions. A comprehensive consultation and assessment are necessary to establish a diagnosis. Our team is committed to assisting both current and prospective patients in navigating and managing the paperwork required for their condition.
Individuals with mental health disabilities often face challenges in accessing benefits beyond those provided by insurance or government programs. The support of your treating mental health professional is essential, as they provide the primary documentation regarding your functional capacity and work-related abilities.
Assessments and endorsements from your treatment providers are key factors considered by Social Security and insurance companies when determining whether an impairment significantly affects your ability to work.
As part of our services, Gotham Psychotherapy assists patients with disability paperwork. We ensure documentation is accurate, valid, and aligned with all requirements.

You will coordinate with your individual psychotherapist at Gotham Psychotherapy to schedule an appointment for an initial Psychiatric Disability Evaluation. This comprehensive evaluation addresses your presenting symptoms, mental health history, functional limitations, occupational history, daily life functioning, and safety considerations.
If you remain out on disability following the initial evaluation, Gotham Psychotherapy will conduct a Monthly Symptom Re-Evaluation every thirty (30) days. These follow-up assessments are designed to monitor changes in symptoms, review your ability to manage daily tasks, assess cognitive and emotional functioning, evaluate social and interpersonal engagement, review treatment response and any side effects, and ensure that safety concerns are continually addressed. Each re-evaluation concludes with an updated clinical impression and treatment plan, including assessment of readiness to return to work and overall prognosis.
Appointments are structured to be efficient and typically do not exceed one hour.
